 Here are some helpful hints in registering your child: 

  • If this is your first time registering for a sport or activity, choose the “Create New Account” icon. 

  • If you have previously used this site to register for a sport or activity, choose "Login to My Account"

  • When entering the student’s name, it must be entered as it appears in PowerSchool.  

  • When creating a new account, if the student’s name is hyphenated, just leave a space between the names instead of using a hyphen. 

  • Once logged in, complete the required fields. The system will let you know if you’ve missed anything. Once the registration form is completed continue to the next page where (if you are a new user) you will create your login and password for your family account.  

  • Through this family account, you can upload your medical forms and will always have access to them.  You will also receive your child's clearance through the family account and from the Nurse's Office. 

Frequently Asked Questions: 

  1. Coaches will send out practice schedules to prospective athletes in their respective sports.   

  1. Updated Medical Forms must be completed annually. These can be obtained in the Athletic Office or by clicking the links above for each form. 

  • The "Preparticiption Physical Evaluation Medical Eligibility Form" is the cover page for the Sports Physical Packet and is one page in length. This form is completed by the Healthcare Provider, must include their Office Stamp and is returned to the WDHS Nurse's Office. 

  • The "Physical Evaluation History Form" is part of the Sports Physical Forms and is two pages in length. This form should be completed by the parent or guardian. This form is maintained by the Healthcare Provider and NOT shared with the school.

  • The "Physical Examination Form" is part of the Sports Physical Forms, is two pages in length and must be completed by the student-athlete's family physician. This form is maintained by the Healthcare Provider and NOT shared with the school.

  • The Asthma Medication Form, Food Allergy & Anaphylaxis Emergency Care Plan and District Self-Medication Permission Form are part of the Medical Forms and should be completed if the student-athlete has asthma  and/or allergies and is prescribed an inhaler or epi-pen. These forms must be completed by the parent/guardian and the family physician. Once completed, these forms are turned into the WDHS Nurse's Office.

  1. The ImPACT Test is a computerized baseline concussion test. The online ImPACT Baseline Test is required once a year during your freshman & junior years before the athlete’s first sport. If you are going to be in 10th or 12th grade and this is your first time on a sports team, or you are new to West Deptford High School, you will need to take the baseline test. If you are currently being treated by a doctor for a concussion, do not take the baseline test and contact the Athletic Trainer or nurse ASAP. 

  1. Current Asthma Action Plans, Emergency Care Plans, and Self-Medication Forms will expire on June 16th.  These forms are good only for the school year.  They expire after the last day of school. If your child has one of these plans, the current plan must be on file with the nurse. 

  1. All athletes must have a current physical on file as well as a current Health History Questionnaire, which must be completed before each season. Physicals are good for 365 days.  

Per state regulation N.J.A.C 6A: 16-2.2 school/team physicians must approve all NJSIAA State physicals for athletics. Therefore, we must have the forms completed and submitted a minimum of one week before practices begin as there is a five to seven-day turnaround for physicals to be approved.
